Start by using a FTP program like Filezilla to connect to your site and copy your theme files.

Next get recent copy of your updated theme.

If you know the changes you made you can copy and move them to the new file.

If you don't remember everything just install a file comparison software.

WinMerge is for Windows and Kaleidoscope is for mac.

You can then compare the changes add move over what you added to the new theme.

Finally upload your updated theme.
